When you board an airplane, you do not often check on the pilot's qualifications or personality. You assume (or hope) that he or she are qualified and experienced. Choosing a physician for medical treatment  is more complicated. This is not only a question of whether your physician can operate complicated machinery but also whether he or she can do it with the least amount of risk and expense and the greatest amount of care and respect. Even when it comes to a medical subspecialty such as reproductive Endocrinology and Infertility (REI), where there are less than 1000 board-certified physicians in the U.S., you still have a choice as to where to receive care. When considering an REI subspecialist, then, please ask the following questions: 1. Is the physician qualified and well-trained? 2. Does the physician care about his/her patient? 3. Does the physician relate well to his/her patient? 4. Does the physician work in a qualified facility and has good support staff? My response: 1.  I am one of 4 board-certified REI specialists in the State of Oklahoma. I have more than 20 years experience in REI and 24 years experience in obstetrics and gynecology. I received my general OB/GYN training at an excellent residency training program (University of Alabama Hospitals) and my subspecialty fellowship training at another prominent program (University of Louisville). I served on faculty in Louisville and at the University of Oklahoma for 7.5 years, which included academic activity such as teaching and research. I renew my board certificate every year in both general obstetrics and gynecology and REI and constantly keep up with current literature and continuing medical education. I served as the director of the ART program at the University of Oklahoma for 5.5 years and at Bennett Fertility Institute for more than 13 years. 2.   I care a lot about my patients but will leave this section short- to flaunt compassion and humanism has the same credibility as self-advertising. The vast majority of my patients, though, realize from the start that the terms "care" and "compassion" ring very true and are the cornerstone of my practice. 3.   Rapport with patients is of paramount importance to me. This consists of respect, tolerance, a listening ear, humor, and strict adherence to confidentiality and ethical integrity. I come from a different background than most and speak several languages. Having traveled extensively, I understand and respect diversity. All my patients have my pager number and can call me 24/7 in case of an emergency or urgency. I see patients every day, including weekends and holidays. In my practice, we try to be flexible and accomodating in terms of hours of operation and respect for the patient's own wishes and anxieties. I have two roles: as an REI specialist in private practive, and as the medical director of Integris Baptist Bennett Fertility Institute (BFI). My office and BFI are right across the hall from each other. I have had one nurse, Beverly, in my private practice for 13 years, ever since my arrival at Integris Baptist Medical Center. At BFI, I am in charge of 3 nurses. All four nurses are spectacular in terms of clinical knowledge and compassion. BFI's virtues are described below. How do the pregnancy rates compare with the national standard? 7. Are these pregnancy rates reported in an honest and transparent way to the patients? Facts and figures on BFI: 1.   BFI is an original member of the Society of Assisted Reproductive Technology (SART), a subspecialty organization of the American Society of reproductive Medicine (ASRM). To be a member of SART, a program must meet strict staff requirements; perform a minimum number of assisted reproductive technology (ART) procedures per year; and maintain documented live birth rate of at least half the national average. BFI has consistently outperformed the national average in the past 10 years. Reapplication and certification is required every 3 years and BFI has been recertified successfully in each year of application. The BFI culture laboratory passed the strictest criteria for certification by the College of American Pathologists (CAP) in each of its required years as well as a stringent examination by the Food and Drug Administration (FDA). BFI is a "Center of Excellence" for Integris Baptist Medical Center. It is JCAH-accredited at both the clinical and laboratory levels. 2.    BFI's program director and founding member is Dr. David Kallenberger, an OB/GYN with 30 years experience in complex gynecology and infertility medical and surgical treatments. Its medical director is Dr. Eli Reshef, a board-certified reproductive endocrinologist with 20 years experience in REI. The IVF laboratory director,  J. Edward Wortham, PhD, HCLD, is one of BFI's original founding members (established 1985) with more than 30 years experience in high-complexity laboratory directorship. He also serves as a national examiner of other ART laboratories. BFI has 3 nurses with combined experience of more than 60 years. Our nurse supervisor, Cindy Penland, has been at BFI since 1986. 3.    BFI has consistently received some of the highest scores in patient satisfaction surveys at Integris. The patients interact freely with the physicians, nurses, and laboratory personnel. At any given day, weekends and holidays included, there is a physician, a nurse, and a laboratory technician on the premises. Unlike most IVF programs, our laboratory personnel directly interacts with patients, whether by phone or in person. Our nurses are one the secrets for our success at BFI, and this is no empty slogan. Besides being genuinely nice, they are caring and competent and very experienced and sensitive to each patient's unique needs. Consequently, the complicated and taxing IVF process has been made much easier for our patients, with very low incidence of confusion about medications or dissatisfaction with care. IVF is a complicated and stressful journey. Our nurses alleviate much of the extra stress that is often detrimental to success. We do our best to be accomodating to our patients in terms of appointment time. We also try our best to reduce the wait time for each patient. 4.   BFI is open 7 days a week, 365 days a year. IVF procedures are performed 11 months a year, with a month break in mid-December, mid-January for cleanup and extensive quality control (our Winter "Spring cleaning"). Inseminations are performed every day, including weekends and holidays. Most hormone blood levels are performed daily. Semen analyses are performed Monday through Friday and the results available to patients and their referring doctors the same or the next day. I am not aware of many clinics, with the exception of emergency clinics, that are open as many hours as BFI. 5.   BFI is an advanced reproductive care facility with state-of-the-art equipment and procedures. We have a very large and modern laboratory with some of the most advanced instruments in the field, including laser for assisted hatching (AH). Other than conventional in vitro fertilization (IVF), we perform intracytoplasmic sperm injection (ICSI), microepididymal sperm aspiration (MESA), egg and embryo donation, embryo and sperm freezing and storage, and artificial insemination with husband and donor sperm. BFI does not provide surrogacy services. Preimplantation genetic diagnosis and screening (PGD, PGS) is not provided due to the lack of clear indications for these procedures and questions about their efficacy and benefits. If any of our patients need PGD/PGS or are interested in any other services that we do not provide, they are referred by us to other qualified clinics. 6 & 7.   Don't take my word for it- simply go to www.sart.org to review BFI's statistics from the past several years. Our success rates in IVF have consistently placed us in the top 5-10 percent of all ART programs in the U.S. Moreover, we attempt to place less embryos per patient to decrease the rate of multiple pregnancies. We do not "cherry pick" the best prognosis patients for IVF to inflate our success rates. We essentially take all comers through the ART process, though those with poor prognosis get extensive counseling about alternatives. We emphasize in our results the live birth success rate per procedure. We do not include  positive pregnancy tests that did not culminate in a live birth or miscarriages when we report our success rates to our patients.
